Implementasi Puskesmas Care Call Berbasis WhatsApp untuk Continuity of Care Pasien Kronis

Penyakit kronis membutuhkan tindak lanjut berkelanjutan karena keberhasilan pengendalian penyakit sangat dipengaruhi oleh kepatuhan kontrol, kepatuhan minum obat, dan kemampuan pasien melakukan perawatan diri. Di Puskesmas Perumnas Arga Makmur, hasil analisis situasi menunjukkan belum adanya sistem follow up terstruktur bagi pasien kronis, sehingga diperlukan inovasi sederhana berbasis teknologi komunikasi yang mudah diterapkan. Artikel ini bertujuan mendeskripsikan implementasi Program Puskesmas Care Call berbasis telepon dan WhatsApp dalam mendukung continuity of care pada pasien penyakit kronis di Puskesmas Perumnas Arga Makmur. Metode yang digunakan adalah laporan evaluasi program deskriptif dengan pendekatan kuantitatif dan kualitatif sederhana, karena desain ini sesuai untuk menggambarkan proses implementasi, capaian indikator, serta pembelajaran awal dari proyek inovasi. Program dilaksanakan melalui pendataan pasien, sosialisasi, komunikasi Care Call satu kali per minggu, dokumentasi, monitoring, dan evaluasi terhadap enam pasien kronis selama periode monitoring Mei 2026. Hasil evaluasi menunjukkan bahwa seluruh pasien berhasil dihubungi 100%, kehadiran kontrol mencapai 83,3%, kepatuhan minum obat mencapai 83,3%, pasien putus kontrol sebesar 16,7%, dan kepuasan pasien terhadap program mencapai 94,4%. Program Puskesmas Care Call berpotensi menjadi strategi low cost-high impact untuk memperkuat kesinambungan pelayanan pasien kronis di Puskesmas. Penelitian lanjutan direkomendasikan menggunakan jumlah sampel lebih besar, durasi pemantauan lebih panjang, dan indikator klinis seperti tekanan darah, gula darah, serta kejadian rujukan.
